This patient presented with concerns about loose skin and large saggy implants. She preferred no implants. I removed her implants and all scar tissue (capsulectomy) and then did a breast lift. A very nice outcome and a small B cup breast.

Women may want their implants removed for various reasons: implant capsular contracture, which leads to pain, malposition, and poor aesthetics, bottoming out, increase in existing breast volume overtime, or fears of breast implant related illnesses.  The management of implant removal may involve “en-bloc,” capsulectomy, total capsulectomy, segmental total capsulectomy, or capsuloaugmentation lifting and even fat grafting.  All done to improve the aesthetic outcome in complex situations.

Unexplained medical issues will often lead patients to remove their breast implants.  Defects left behind from removal of implants (explantation) and capsulectomy, can lead to significant cosmetic deformity.  My solution includes developing dermoglandular flaps from the saggy tissue which fills in the voids with a breast lift (non-implant augmentation).  A fine result.

She did not like the shape (“bottoming out”) and double bubble.  She did not want implants any longer.  I did total capsulectomy and combined with auto-augmentation (non implant) breast lift using multiple internal dermoglandular flaps.  A very nice outcome.
