Spiro and colleagues examines an advanced limb-salvage approach for patients with severe Grade IIIB and IIIC distal tibial injuries. Five patients with extensive soft-tissue and bone loss underwent lower-extremity reconstruction combining microsurgical free-tissue transfer with bone transport through distraction osteosynthesis. At follow-up ranging from 17 to 34 months, all five patients were walking, fully weight-bearing, and free of pain, without requiring below-the-knee amputation. The study highlights the role of coordinated plastic surgery, microsurgery, and orthopedic reconstruction in preserving function after complex traumatic leg injuries.
