In my version of a Deep Plane facelift, I use the high SMAS approach and separate the layers of the skin away from the SMAS (called bilaminar) as I enter the deep plane. From here, I can access buccal fat as needed to trim modestly (not severely), and I can rearrange tissues more naturally than with the Composite Monolayer Deep Plane technique. In this case, I combined it with upper and lower eyelid blepharoplasty, lower eyelid fat bag transposition to the cheek, and dermomuscular orbicular pennant (lower eyelid muscle tightening and re-suspension). I can lift jowls virtually into the cheek. A beautiful transformation!
