Eyelid Surgery Cases

This patient underwent combination upper and lower eyelid blepharoplasty with lower eyelid fat transposition (moving periorbital fat to nearby locations), and mini lateral cheek lift (utilizing technique of DMOP (dermo muscular orbicular pennant)) support. I also did fat grafting of multiple sites to face including cheeks, jaw line, chin, and marionette folds.  All together this creates a dramatic youthful transformation.  A very fine outcome.

This case highlights my technique for managing periorbital aging particularly in lower lids.  In order to properly address all aging components; skin excess, fat bags, falling cheek, loss of cheek volume, and lid cheek junction hollow, I separate out the three main fat bags, release the lid cheek junction attachments so this saved fat (not discarded!) can be transposed (moved) into the hollows.  I then inject this fat with fillers into the cell mass under direct visualization (distinct from in-office filler and far more precise and long lasting) and then I perform retinacular release to aid in mini-lateral cheek lift which is elevated into a higher position by dermo-muscular-orbicular-pennant flap (DMOP).  The results far exceed typical more simplified eyelift “lift” procedures.  An outstanding outcome!

This patient presented with the typical combinations which lead to facial (peri-orbital) aging, including skin laxity, volume loss, and sag. The treatment included release of fat bags, transpositions of fat across cheek bone, lifting of lateral cheek back up over eye socket, and finally skin excision. An excellent result.

This patient underwent upper and lower eyelid blepharoplasty to correct excess skin and fat bags around the eyes. I also did a cheek lift through the lower eyelid incision and re-positioned the eyelid fat to fill the lower lid/cheek hollows. She is shown 12 weeks later with a very youthful appearance and a fine outcome.

Eyelid procedures are not just about fat removal.  In fact, I remove very little fat.  I often add fat (fat grafting) or fillers in addition to repositioning/redistributing the lower eyelid fat (fat transposition) over the cheekbone at the lid cheek junction.  In addition, I elevate the lateral cheek complex over this fat redistribution.  It is a true volumizing procedure and together with a modest skin removal yields a true youthful appearance.
