This patient presented years after her initial breast reconstruction done by another surgeon. She was unhappy with the poor camouflage of the mastectomy and the poor shaping of the reconstructed breast and the implant in the opposite breast. I removed and replaced both implants, reshaped both breasts, added a tissue graft (alloderm) to the right breast, did a lift, and then finally 3D tattooing and fat grafting. A fine result.

This young woman presented to me after an attempt at breast cancer reconstruction (after mastectomy) from another institution. She was unhappy with the poor shape and incomplete camouflage. I reconstructed her by re-configuring the implant space – creating a more natural curve, changed the implant, made a nipple and areola (also pigmented), fat grafted the perimeter and over the implant and did an augmentation to create balance. A very fine outcome, and a grateful patient.

This young woman presented with single unilateral breast cancer. She chose a unilateral left mastectomy. Reconstruction included a staged approach first with a tissue expander and allograft. Subsequent second stage surgery three months later included the removal of the expander on the left, shaping of the space, and placement of a silicone implant. The right breast had a lift and augmentation done at the second stage for symmetry. Finally, nipple reconstruction and fat grafting to the interface between the upper mastectomy and the implant completes her fine outcome. (Pending tattooing of the nipple.)

This case represents the all too common occurrence of post-mastectomy secondary deformities after implant reconstruction. This patient came to me after her reconstruction was done by others at another institution.

The residual problems include the small and malpositioned nipple reconstruction. The implants are flat and lack projection.

The treatment involved two stages. First, reshaping of space (capsuloplasty/capsulectomy) and placement of a high profile gel implant. Later, fat grafting and a new nipple reconstruction completed the outcome. A dramatic transformation

This patient presented for a bilateral mastectomy. She had a strong family history of breast cancer. A staged approach with tissue expanders, followed by implants and then nipple reconstruction and fat grafting has yielded a very nice outcome. (Awaiting pigmentation.)
